Who We Are

The Greater Cleveland Community Health Workers Association is created and ran by all certified Community Health Workers and other health professionals.
GCCHWA was created to bridge the gap between CHWs throughout Greater Cleveland serving Cuyahoga, Lake, Lorin and Medina counties.
Our aim is to address the needs of CHWs, support work efforts, provide mentorship and training and increase opportunities for Community Health Workers.

The Cuyahoga County Board of Health, A Vision of Change, Creating Greater Destinies and the ADAMHS Board have teamed up to launch this video series titled Our Stories, Our Healing as part of an Adverse Childhood Experiences (ACEs) grant from the Centers for Disease Control and Prevention (CDC). This series aims to raise awareness through storytelling by community members who have overcome traumas and found healing. Mental health, substance use disorders and domestic violence are a few of the topics that will be explored throughout the series.

A Vision of Change's Community Health Home
Renewing our Pledge to Ms. Cassie's House i!
Since closing and officially obtaining the first property in our Community Village to Wellness initiative on September 14th 2021, A Vision of Change has been utilizing Mrs. Cassie's House, as the neighborhood location where a collaborative array of services, programming, health screenings, and culturally relevant activities and celebrations can take place to promote resident driven services to impact health disparities.
Though the first steps have been taken, there is still a lot left to do!
We ask for your support in making this resident driven heath home a reality.
